About Leukemia Panel, ALL, By FISH Test

The Leukemia, ALL, Interphase Fluorescence in situ hybridization (FISH) Panel is designed to diagnose the most common and prognostically significant chromosomal abnormalities associated with acute lymphoblastic leukemia/lymphoma. The test identifies rearrangements in specific genes used in risk stratification and treatment decisions for children and adults newly diagnosed with ALL. It is also used as a complementary test to conventional chromosome studies when the sample is inadequate for analysis, following an abnormal clone, or assessing relapse and progression.  

Acute lymphoblastic leukemia (ALL)is a type of blood and bone marrow cancer that progresses quickly and creates more immature white blood cells called lymphocytes. Approximately 70% of all childhood leukemia cases (ages 0 to 19 years) are ALL variety, making it the most common type of childhood cancer. With early detection and treatment, the chance of cure in children is high.

ALL also occurs in adults, although the chance of cure is significantly reduced.

Test preparation

To do a FISH test, your doctor may need your bone marrow samples (either tissue biopsy or fluid), which will be collected during bone marrow aspiration or biopsy procedure.

Before the procedure, ask your doctor whether you should avoid eating or drinking ahead of time or stop taking any medicines or supplements. Make sure your doctor knows the history of your medical conditions and allergies.

Interpretation of Test Results

The results of the ALL-FISH panel are interpreted as follows:

Positive result

This report suggests rearrangement/translocation or changes in copy number of BCR-ABL1 t (9;22), KMT2A (MLL), ETV6-RUNX1 t (12;21), and E2A (TCF3), indicating the diagnosis of ALL.  

 

Negative result

This report indicates the absence of the above-described rearrangement/ translocation in the specific chromosome location.

In Leukemia, ALL FISH testing, the lab technician will utilize a short DNA sequence known as probes tagged with fluorescent dye. These probes bind to the complementary DNA stretch on the chromosomes in tumor cells. When exposed to fluorescent light, they glow, indicating their location on cancer cells and pinpointing chromosomal abnormalities that may present.

This panel can detect only specific chromosomal aberrations targeted by the FISH probes included. Any aberrations outside these regions will not be detected by the probe.

The results of this test are highly reliable if the test is conducted in an accredited laboratory, with interpretation done by a qualified healthcare professional.

ALL FISH testing may take anywhere between 2-5 days to generate a result.
